What the community library is

On the Templates page, the Community section holds templates published by other Carepatron practitioners: mental-health intake forms, physio assessment sheets, dental check-up checklists, coaching goal-setting templates and more. Click View all to browse the whole library.

Everything in it is free to use. It's a shortcut past the blank page, starting from something a peer has actually used.

Find and try a template

Search by keyword ('intake', 'SOAP', 'PHQ-9') or filter by profession, tags, collection or Auto-scored. Each card shows its collection, its author and how many times it's been used. Preview a template, then choose:

  • Use template starts a note with it, without saving it to your workspace.
  • Copy to workspace adds an editable copy to your team templates.

To send a Community template as an intake form, copy it to your workspace first.

Customise after copying

Once it's in your workspace, treat it like any other template. Open it, change the wording, add or remove fields, drop in your smart chips, save. The community version isn't touched; you've made your own copy.

Keep what works, and replace the rest with your own questions and wording.

Publish back, if you want

Templates you've built and refined can go back to the community: open the template's overflow menu → Publish to community → pick a collection and add a short description.

Treat the library as practitioner-contributed. Always preview a template before using it on a real client, and check it meets your clinical and regulatory requirements.

Check it worked

Tick these off in your own workspace before moving on.

What's worth borrowing

Five community templates worth starting from.

A few good places to start. Search for these by name.

PHQ-9 + GAD-7

Depression and anxiety screens to send as regular outcome measures. Use the Auto-scored filter to find versions that score themselves.

CBT thought record

A worksheet to use as homework between sessions or as part of intake.

Initial physio assessment

Pain, range of motion and history in one assessment you can adapt.

New patient dental intake

Medical history, anaesthesia and anxiety questions. Adapt it for children or adults.

Coaching discovery call

Goals, constraints, motivation, schedule availability. Send before the first session, save the answers to the client record.
